Self-Reliance Plan for Members

Self-Reliance Plan. Members use this form on their own or with the assistance of a mentor. The form helps members assess their needs, income, and expenses; identify available resources; identify service they can give in return for assistance received; and develop their own plan for becoming more self-reliant.

Members ́ Level of Functioning and Guidance on how to work with them

Consider your total experience with this member. How well is he/she functioning at this time?
Include the member ́s emotional, social, and occupational functioning, with regard to mental health.

When Do We Seek Professional Help?

In general, there are three keys to determine whether a problem can be solved without professional help or if such help is needed.

Welfare Services Referral Portal Instructions for Bishops

There are several ways to get to the Bishop's Order and Referral page. This is where you would complete
a food order, however, it also allows you to refer a member to Family Services for counseling.
