This document includes general guidelines and statements from Church leaders to support
those guiding the preparation and assessment of missionary candidates. LDS Family Services
can consult on specific cases or complete a pre-mission assessment.

Self-Reliance Plan. Members use this form on their own or with the assistance of a mentor. The form helps members assess their needs, income, and expenses; identify available resources; identify service they can give in return for assistance received; and develop their own plan for becoming more self-reliant.

Consider your total experience with this member. How well is he/she functioning at this time?
Include the member ́s emotional, social, and occupational functioning, with regard to mental health.

In general, there are three keys to determine whether a problem can be solved without professional help or if such help is needed.

Family Services assists leaders to help members develop emotional self-reliance
and overcome obstacles to making and keeping sacred covenants.

Bishop’s Guide to the Self-Reliance Plan. Bishops and branch presidents use this form to plan and track welfare assistance and to follow up on members’ personal self-reliance plans.

There are several ways to get to the Bishop's Order and Referral page. This is where you would complete
a food order, however, it also allows you to refer a member to Family Services for counseling.
